Just wanted to close this thread out. Thanks to all that responded, I really appreciate it. The issue was the studs and the rotor. It looked like the studs that had been replaced at one time in the vans life before I owned it (it spent some time in Mexico) were not correct. They were a few mm's off and were not seated correctly on the rotor. Thank god that nothing ever happened! A new rotor from Quigley, and new studs, and the wheel is tight and to spec. I was very very lucky.
