Nothing to do with vans but I saw this on a welding board I frequent:

BAM. with me in the truck. i lost all hearing out of my right ear and got a scratch on the back of my head. all things considering, im alive.

There is a reason why you should never move a gas bottle without the safety cap in place. Now if only I could remember what that reason is.
