Wireless Keypad Programming Instructions

My wireless keypad did not come with instructions, and after trying six different ways, based on posts here and YouTube videos--and failing to get my keypad to program--I dug around the Internet until I found the official instructions, which I am attaching to this post (in two pieces, due to forum document size limits).

What the instructions do not say is that in some vehicles it is necessary to shut all the doors before starting the procedure. Also, if programming fails, try one or more of the following:
- lock all of the doors before starting
- unlock all of the doors before starting
- open and close the driver's door to reset the system
- let the keypad sit idle for five minutes

I tried all of these things at one point or another. Eventually, I hit the right combination, and the keypad accepted the programming.
