Awning Window Handle and Trim

I've been searching without success, so hopefully somebody can point me in the right direction. Started putting walls up in my van and realized the handle to open the awning window will be way to short. This first picture is the best I found to show what I'm talking about (stolen from Skinny's build):

You can see the crank shaft, but there needs to be some sort of extension like you can see in this picture:

Rather than mess around making one, I was hoping there was already something available but I've been unable to find one or a thread that even mentions it.

The search brought up another question, how does one come up with such a trim for the window like this one Skinny did:

I was just going to make a rectangular one, but this one looks WAY better than what I had going and I've seen others like it as I've dug around for the handle as well. Looks like he built it, so I was digging for details on how he or others have built similar but am coming up short. At one point I thought I'd seen a injection molded trim like this that could be purchased, but now I can't find it, either.

I ordered a new awning window crank from Peter at SMB west a few years ago, I believe the assembly comes with the extension piece.
