My 06 Quigley V10 with 3.73 and 265/75-16s, no PH, no Thule bars, stock bumpers got 15mpg from San Diego to Cupertino. About 475miles and I managed to stay around 55-60 the entire time. Tires were pumped to 50psi.
Once back to Cupertino and entered into service (going to work, dropping off the kids, going to the grocery store, etc.) I get about 10mpg.
My other 06, same gears, with PH, 2WD, 5.4V8, 245/75-16s not loaded down gets about 2 mpgs better.
Having a profile like the side of a barn doesn't help ..

Man, this would be a great opportunity for someone doing research to go through this entire thread and get an average of the averages! Heck, if I get bored at work in the next couple weeks I might do it myself. It wouldn't be hard and fast data without having everyone's total mileage to do a weighted average, but still a pretty good idea of what is going on.
I'd say there are enough samples for 7 categories:
5.4, 2wd
5.4, 4wd
V10, 2wd
V10, 4wd
7.3L, 4wd
6.0, 4wd (not large samples of 2wd for either diesel I would think)
Sprinter (all)
I would probably have a City and Hwy column for each of the above too.

I was just joking, but it seems like such a straightforward question elicits a lot of emotions and especially confirmation bias.
Here is my actual experience: 13.5-(my Ford 6.0 RB 4x4, 4.10). 15-(my Chevy EB Duramax 4x4,3.73). Those are both gazole (so it's not a g@s vs. d13s3l thread). YMMV.

I've had a chance to track my mileage a bit more. On a "solo" (i.e. not towing) out-and-back highway trip in moderate weather earlier this month (I-5), I got about 16mpg on the highway. I was not grossly speeding, but was "pushing it," so no lollygagging. i.e. if the speed limit was 70, I was going 71-72, and not 65-70. The van was reasonably loaded with gear, but the tanks weren't full.
I got 15.9 to 16.9 on the tanks that covered non-mountainous stretches (the high number included some two-lane highway so I was going more like 60 mph), and around 15 on the tanks that included Siskiyou Pass and environs. This is with my 1990's SMB E-250, 5.4 gas, 2wD, PH, stock-sized tires. Nothing extra on the roof.
This summer I took a long trip (NW coast to Great Lakes) in a friend's F-350 dually 4-door with V-10 (4wD but just stock tires) and cab-height smooth topper. Nursing it along on the way out (60 mph or so), we averaged 13.8 mpg. This was an all-time best. When we pushed it up to 70mph+ that went down (I forget to what though.... probably 11-12 if memory serves).
On the way back we were towing a ~8,000# travel trailer, and we got around 9 mpg.
I know none of this relates directly to a lifted 4wD SMB with a V-10, but maybe it's helpful nonetheless. Just an "off the cuff" estimate from me: the V-10 seems like it "costs" around 2-3 mpg over the V-8 (5.4), all else being equal (?)
I've been thinking I'd prefer a fixed top (16") to my PH top; figure that might lop off 1-2 mpg, especially in wind or at 70+ mpg, although I'm not sure of course.

I just did an 800 mile ThxGvng trip and got 15 running 70, with the 2wd 5.4 van, half loaded. Best I've ever gotten, apparently you DO get better mileage if you slow down.

Quigley 4WD, V8, 5.8 liter, PH top but passenger interior, keeping up with speed limits and not worrying about having a light foot: 12 mpg
Same van, driving gently and keeping to a max of 65: 12 mpg
I think I did see an average on a tank of 13 mpg once but I'm thinking it was a maths error...

Just bought a 2013 Chevy Avalanche. I like the new technology for increasing gas milage. This truck shuts down four cyclinders, going from V8 to V4 based on power demand. The changeover is rapid and unnoticable were it not for the dashboard information center. Gets 21 mpg highway.
P.S. The van is 2WD, so I needed to fix up a 4WD for winter snow activities. Plus, the van only carries passengers facing sideways and most don't like to ride this way, so the full cab gives me a way to take friends along.

2007 Quigley V-10
Came from the factory with 3.73 gears. Running on LT285/75R17 tires. Mileage - 12 mpg.
Because it shifted down every time it came to a hill, but primarily because I drive mainly dirt roads at speeds where the tranny was constantly trying to make up its mind whether to run in direct or in O/D, I changed out front and rear gearsets to 4.56 with ARBs. Result after ~15,000 miles - still 12 mpg! It doesn't seem to matter whether I'm idling along a dirt road at 45 or flying down the freeway at 75, it still gets 12 mpg. But I'm glad I changed the gears because it's made the van a much better rig in the mountains and in the rough going.
