Tell us more and we'll tell you more. 181k is not bad for any E-series van though. Fleets run them hundreds of thousands of miles. If it is a 5.4 v8, 6.8l v10 or 7.3 diesel you should be fine.

I had a 5.4 in my last van, a 2002 E350. I'm thinking that is probably the engine in the one you're looking at. I had an engine failure (rod bearing) at about 140k, but that was VERY unusual. ...
