Either rust from a leaky EGR cooler, or carbon buildup from a faulty EGR valve. Sometimes however the turbo vane positioning solenoid can go bad too, but that wouldn't normally be suspect unless you...

Or they're stuck in the middle, so its out of spec on both ends.

Beware... Find the root cause first. Just swapping out the turbo means you'll get to do it all over again after not too long....

It is OBD-II compliant but will only show emissions related code. Many, if not most, codes will not be read by a generic OBD-II reader. You need a Ford-diesel specific reader.
