Well, my van was old when I got it (95 purchased by by me in 2012 or so) and it was much older technology (460, older Transferflow front and rear tanks I replaced with a single Aerotank designed for a diesel van.
My take was that the Aerotank just didn't work well with the vents and canister on that van or that I had a blockage or leaky vent/line that I never found. It only happened when I filled it to the brim, so that could've been a big part of the problem too.
