Having severe wobble/vibration on braking on my 2006 Quigley. Is this loose fit in the bushing in the center of the picture most likely my issue?
From what I can tell that is your swaybar bushing. Although it does appear a bit loose,I really doubt that is the issue. I would check the track bar bushings for play, as well as all the joints in the steering system.

Is that kind of play normal on the sway bar bushings? There is actually twice as much play as the photo shows. I could not get the camera high enough to show how much there really was. Should I be replacing those with new ones?
Play in the sway bar bushing doesn't matter much, it at all. After an inch or so of travel the sway bar will come into play.
To check the steering, have someone turn the wheel back and forth HARD while you are underneath checking for movement. And watch that track bar at both ends.

I have a 09 quigley with just under 5000 miles mechanicaly completly stock I hit a patch of uneven asphault road at 50 MPH and experieced DW I eased on the brakes and it got worse I coasted to about 25 MPH and it stopped. I was sure that some kind of damaged had happened. After checking and driving for about 1000 miles since no other problems have occured. So I don't beleive after market parts or wear are the main cause. When you have a vehicle that has been mechanicaly converted you are taking a chance that things aren't as good as a factory stock setup. I go over that same stretch of road on a regular basis and the DW only happened that 1 time. It may take a long time before the DW factors line up again so diagnosing a cure is very difficult. I am going to try bilsteins to improve the ride and hopefuly help with the DW.
You might consider an extra steering damper or aftermarket dual damper set up. It's not likely to be your existing shocks if they only have 5000 miles on them. I assume you are coil spring rather than leaf.
After a few PMs and emails, I was persuaded to post this update.
I have since put several thousand miles on the E350 and have not experienced a hint of death wobble. Some people were asking about Bilstein Shock part numbers for use as dual stabilizers.
It's not about which shock you choose for the steering stabalizer, it's the need for an additional damping provided by an added damping set up. Since I added the custom made damper by Quadvan I have had NO further wobble issues. Engineers ad nauseum have spoken about a TRUE correction of the problem by redesigning your entire front end. Buy a steering damper and install it. It will solve your problem.
