This may be silly but....

If the objective is to get the bikes to swing out of the way so that you can use the rear doors, couldn't you get a rear tire hinge mount, and weld the bike rack to that? I'm thinking something that would end up like the passenger side of the Baja box/bike rack combo, but without the box.

or maybe not...

If you have a rack or that is easiest, why not.....

You would have to vertical mount like the over ladder rack or extend to clear the spare. Also the Aluminess releases are hard enough to reach & trigger without going under bikes...

This has worked pretty well for me (if you don't mind not opening your rear doors)...... Plus, it affords a lot more storage with the Jobox..

this should show a few pics of the rear bike rack i made not long ago, it works well and i now have full bike tray racks on that allow me to hold three bikes with front wheels on. im very pleased with it so far, i just hope the door hinges hold up longterm. used some old thule mounts i had sitting around and rectangular bars. nice to have the normal easy access to the rear of the van. used the rear ladder from smb west, seems really solid.
