Well I apologize for wasting everyone’s time! I am a dope, truly. I traced the small wire out and come to find out, it was me, I added the third wire back when I installed my step sliders. I have zero recollection of doing so but sure enough that is where I pulled 12v from for the step sliders. Thanks for all the suggestions of what it could have been.

With regards to the outlet type, this one is worn out and the nubs don’t stay inserted anymore. I have another generic round port back by the sink and 2 up on the dash, so there is no shortage of them. I also found hardwired units that show the voltage and have 3 charging ports like this one here that I liked.

Plus my son sits in the seat near this wall and plugs his crap in, and the nub always seems to find its way into random spots in the van so it seems easier to just hardwire the outlet for now. I can always go back to a generic one if need be.

The upside on those I posted is that they are flush mount and would not take up space you might not have.

For example, you could attach these to your dash with just a couple of mounting holes and a single hole for the 2 power wires without cutting anything larger for a 12V adapter.
